Launched in May 2023, Path-Now has dedicated itself to being an invaluable resource for individuals and families in the intellectual and developmental disability (IDD) community across California. Our mission is to connect users with custom-matched resources and services that meet their unique needs, and in just one year, we have made significant strides toward this goal.

Connections Made:In our first year, we have successfully facilitated 50 meaningful connections between individuals and the resources they need. Each connection represents a step forward in improving the lives of those in the IDD community.

Service Providers: We are proud to have 9,600 service providers on our platform. This extensive network ensures that our users have access to a diverse range of services and resources tailored to their needs.

Review Feature:We introduced a new review feature, enabling users to share their experiences and feedback about the services they receive. This addition not only helps other users make informed decisions but also helps service providers improve their offerings.

Referral Feature: Our new referral feature makes it easier for users to recommend Path-Now to others within the community. By sharing our platform with friends, family, and colleagues, users can help more individuals find the resources and support they need.

On Thursday, November 9th, the San Diego Regional Center (SDRC) buzzed with energy as Path-Now proudly sponsored the Self-Determination In-Action Conference at Park & Market in San Diego, California. The highlight of the event was the innovative Wayfinder Experience, providing attendees with a unique opportunity to test Path-Now and connect with a diverse array of service providers. This interactive session aimed to empower individuals in the self-determination program, encouraging them to make informed choices and carve their own paths to a brighter future.

Nonprofit Adjoin Launches Path-Now, an Innovative Application and Service Provider Network for Individuals with Disabilities

Path-Now connects individuals with Intellectual and Developmental Disabilities (IDD) with thousands of service providers throughout California

SAN DIEGO, May 24, 2023–(BUSINESS WIRE)–Adjoin, a nonprofit, charitable organization that creates limitless pathways for individuals with disabilities and veteran families throughout California, today announced the availability of Path-Now, a new, accessible mobile application that enables individuals with Intellectual and Developmental Disabilities (IDD) to safely and easily connect with community service providers electronically.

Path-Now quickly matches users to service providers that meet their unique needs and preferences. The Path-Now service provider network includes vocational programs, independent living services, transportation options, camps and recreational activities, art programs, and more. The program is currently available in California with 8,000 service providers and the team has plans to expand rapidly.

The application then instantly matches them with organizations that align with their preferences. Users can then connect with their chosen organizations directly within Path-Now to move forward in accessing needed services.

Organizations that provide services for individuals with IDD in California can claim their profile in Path-Now or request to have a profile created. Once requests are received, reviewed, and approved by Path-Now, organizations choose a subscription plan and can review and update information in relevant fields, including uploading photos and videos and gaining access to the Path-Now chat feature.

Path-Now is a new creative, accessible, and unique mobile application that enables individuals with Intellectual and Developmental Disabilities (I/DD) to safely and easily connect with community service providers electronically in California. Path-Now was the vision of the CEO of Adjoin, a 501(c)3 organization, who saw a critical need for there to be an interactive statewide system directory of all community service providers for individuals with I/DD to access needed services.

Path-Now provides individuals with Intellectual and Developmental Disabilities (I/DD), a family member, or a care provider, to create a free profile where the individual can answer questions related to types of service needs, preferences regarding the service or services searching for as well as preferredorganizational characteristics. Then, with the click of a button, individuals can instantly see organization profiles listed in order of percentage of match to individual’s profile responses and read about each potential service provider. Individuals can then choose to connect with organizations by clicking theconnect button for that organization. Based on if the organization has a subscription or not, the individual can either chat directly with the organization or will receive a notification to contact the organization using contact information in the organization’s profile.

Organizations who provide services for individuals with Intellectual and Developmental Disabilities (I/DD) in California can claim their profile in Path-Now or request to have a profile created. Once requests are received, reviewed, and approved by Path-Now, organizations can choose a subscription plan, and based upon the subscription plan chosen, will be able to review and update information in basic fields to all fields, including uploading photos and videos, as well as have access to the Path-Now chat feature.
